The Witcher 3's conclusions
The Witcher 3 has three possible outcomes.
Sad conclusion
Let's move over the terrible conclusion. In the heartbreaking conclusion, Geralt searches for the last crone in Crookback Bog because he misses Ciri's medallion—his only surviving keepsake of his daughter—and learns that Ciri does not survive the events of the game. Naturally, Geralt completes his objective, but the last scene—in which he sits in the crone's hut with his head down and the medallion clutched—is very painful. Monsters are swarming about outside.
You need to accrue three negative points in order to reach this terrible conclusion.
End of the Empress
Recall that getting Ciri to visit Emhyr modifies the way the story ends. Here's how, then. If you bring Ciri to visit her father and Nilfgaard prevails in the current conflict, Ciri will inherit the throne. In addition, you must accrue three bonus points. In this ending, you and Ciri visit several locations in White Orchard, but it's clear that she doesn't want to be there and is aloof (even if she doesn't feel bad for Geralt). After spending some time with her father figure, Ciri surprises him in the end by telling him she is going to Nilfgaard because the throne is the only way she can change the world.
The Witcher's end
You must get three bonus points and refrain from bringing Ciri to meet Emhyr in order to receive the Witcher ending. This ending appears gloomy at first. You tell Emhyr that Ciri died in the last moments of the game, and then you and Geralt go through White Orchard and witness the old Nilfgaardian stronghold. While you wait, a skilled smith emerges brandishing a sword, the inscription on which is up to you.
You have to go to the local inn after this, and lo and behold, there's Ciri! She isn't really gone. Rather, she is prepared to pursue the career of a Witcher. At last, she can be anyone she wants to be.
